Associate Advanced Nurse Practitioner, Band 6 – Community Contract: Permanent | Part time: 30 hours a week | Salary: £36,662 per annum (£40,827 FTE) We are seeking a compassionate and skilled Associate Advanced Nurse Practitioner to join our Community Team. In this rewarding role, you will provide high-quality care and support to people with palliative and end-of-life care needs in their own homes or preferred place of care. Working closely with multidisciplinary teams and out-of-hours providers, you will be responsible for assessing, planning, implementing and evaluating care, while helping to promote clinical excellence and a positive learning environment for healthcare colleagues. Our choir is a cherished part of hospice life, and we are looking for a Choir Lead to guide and grow this community.
