Advance care planning is a process for making decisions about future care. This session will help to increase knowledge, skill and confidence to start and support conversations about Advance Care Planning (ACP) based on individual preferences using engaging interactive methods.

Course Content

• To examine the value of advance care plans to the people within our care
• To explore how and when to commence advance care planning conversations
• To identify potential barriers to having an advance care planning conversations and how to navigate barriers
• To examine the legal framework which underpins advance care planning
• To consider the impact poor planning has on future care

Cancellation policy:

We understand that sometimes it is necessary to change plans. However, to ensure our costs are covered in the event of cancellation the following charges will apply.

• Cancellations (and requests to transfer bookings to another date) made with at least 8 weeks’ notice will incur a £25 administration fee.

• No refund of course fees (partial or full) will be available where notice to cancel is received within 8 weeks of the course start date, or in the event of non-attendance or partial attendance.

Notice to cancel or transfer your booking must be made by email to [email protected].If, due to unforeseen circumstances, Mountbatten cancels a course you will be given the option to transfer your booking or receive a full refund of the course fees.
